Caring For the Whole Person in a Complex Urban Setting

The Family Medicine Residency Program at Community Healthcare Network (CHN) is a three-year residency program for aspiring family physicians operated in affiliation with the Mount Sinai Health System. Our program is designed to create the next generation of leaders in family medicine, whether that means delivering care as a primary physician or working to shape the future of public health policy.

 

Our residency is centered in Queens, New York City, the most diverse borough in the world. The opportunity to treat an exceptionally diverse patient base will prepare you to deliver excellent primary care in a wide range of settings. Our staff physicians are at the leading edge of treating and managing the most common healthcare challenges faced by urban populations, including reproductive health, gender-affirming care, addiction medicine, management of HIV, diabetes, heart disease, anxiety, depression, and many others.

 

As a resident at CHN, you will learn how to work in a team framework alongside care managers, social workers and hospital partners as well as our robust behavioral health, wellness and nutrition, dental, and podiatry programs to provide the holistic support that is necessary to care for our patient population.

About Community Healthcare Network

Community Healthcare Network (CHN) is a not-for-profit, 501(c)3 organization that provides access to quality, culturally competent, and comprehensive community-based primary care, mental health services, and social services for diverse populations in communities with limited access to medical care in four boroughs of New York City. As a network of twelve health centers, a fleet of medical mobile units, and two school-based health centers, CHN reaches over 65,000 individuals each year. CHN serves neighborhoods that are federally designated as Medically Underserved Areas/Medically Underserved Populations.

What does Urban Family Medicine look like at CHN?

CHN’s Family Medicine practice gives residents the opportunity to serve a diverse community. Medical care in this setting requires the practitioner to develop high cultural competency, superb communication skills, and a deep understanding of the broad range of physical and behavioral illnesses among a population often experiencing housing instability, food insecurity, under-compensated employment, stress, and many other day-to-day challenges related to the social determinants of health (SDOH). Upon graduation, residents will go on to become leaders, innovators, and advocates in the medical system who are specialized in providing full spectrum primary care for all age groups in a wide range of healthcare settings.

Hospital and Community Partnerships

The CHN residency program combines the mission driven expertise in ambulatory care of a Federally Qualified Health Center with a world-class healthcare delivery system to provide excellent training opportunities. Through our hospital and community partnerships, CHN residents will be well equipped with the resources they need to provide the highest quality care to their patients.

Frequently Asked Questions

Will I be a resident of CHN or Mount Sinai?

We are a Teaching Health Center. Residents are employed by CHN but train at Mount Sinai and Elmhurst Hospital.



How will I interact with the Mount Sinai system?

As a resident, you will receive your adult inpatient training, geriatric/palliative care, ER and ICU training at Mount Sinai Queens. You will receive your OB, pediatric inpatient training at Elmhurst Hospital. Throughout your residency you will have access to Mount Sinai medical school and many of their resources, and you will receive ambulatory specialty training at Mount Sinai.



When was the Family Medicine Residency Program established?

Our program was founded and accredited by the ACGME in 2024. The first cohort of six residents started on July 1, 2024.
